After slaying the Adyr, you have to make some choices again. But before that you have to kill many NPCs and betray them as well. The major requirements to get the secret Umbral ending are,

• You have to free Gerlinde from her prison in Pilgrim’s Perch
• Deceive Damarose the Marked and exhaust her dialogue 
• Travel to Revelation Depths and slay Harrower Dervla the Pledged Knight. Unlock the well using Umbral Scouring Clump
• Next, go to Mother’s Lull, speak to the non-hostile remnant enemy and also collect the quest item called Damarose’s Seedpod
• Use the Seedpod to betray and slay Damarose near the Shrine of Adyr
• Next, you have to betray Gerlinde and kill her to get another Seedpod
• This will give you the access to reach the platforms in Mother’s Lull
• Next, you have to defeat the Lightreaper in Bramis Castle
• Collect the Umbral Parasite and place it in the available slot in Molhu’s room
• Now, you can interact with Molhu and discuss further about your next targets
• Travel to Skyrest Bridge and Soulflay Pieta
• Complete the boss fight against Elianne the Starved and extract her Parasite 
• Now insert Elianne parasite in the slot in the same room where Molhu is located
